Summary:
Provide hands-on training and direct supervision to staff.
Oversee daily operations in the home.
Support clients within their living environment and community.
Ensure all services are provided to clients within the appropriate plan of care.
Essential Functions:
o Provide support services to clients o Provide training, support, direction, professional tone and cross training for direct support professionals.
o Work with team in developing, implementing and monitoring services in accordance with the Individual(s) plan of care o Review staff service notes for content, consistency, and timeliness o Maintain files pertinent to the on-going operations of work locations o Participate in Quarterly Individual (s) file review o Always ensure Individual(s) safety.
Report incidents/injury/illness in a timely manner o Be available to cover and/or fill in shifts as needed (varied shifts) o Finance management, menus and shopping as needed o Maintain all required certifications o Maintain home in compliance with regulations as well as overall maintenance and cleaning o Train staff on Global Hazard Systems and Safety Data Sheets o Behavioral health/CCSS services o Compliance monitoring (ALH, FCS.
Federal, THERAP, Alayacare, Medications, Evacuation Drills, receipts) o Respond to staff after hours, weekends, and holidays to resolve staffing issues and service concerns as needed o Other duties as assigned ADDITIONAL REQUIRED JOB DUTIES:
o Help FCS meet its quality improvement, evaluation, and reporting o Contribute to developing outcomes and indicators and setting targets o Participate in the collection of data o Oversee quarterly file reviews and monthly data reporting QUALIFICATIONS, EDUCATION AND EXPERIENCE REQUIRED:
o High School Diploma or GED o Basic computer skills, Microsoft Word, Windows, Excel o Ability to solve problems makes decisions, have strong organizational skills, and work with minimal supervision.
o Valid Alaska Driver License and Auto Insurance, Ability to transport clients in your own vehicle PHYSICAL REQUIREMENTS:
This position requires occasional climbing, balancing, stooping, kneeling, crouching, reaching and crawling.
Perform medium work involving exerting 20-50 pounds of force occasionally, or 10-25 pounds of force frequently, or an amount greater than negligible and up to 10 pounds constantly to move objects.
Work is performed in a Group Home and office setting.
LICENSES AND SPECIAL REQUIREMENTS:
Current TB test, CPR and First Aid Certification and Basic/Intermediate MANDT training.
