Summary:
Perform a variety of activities surrounding client care and regulation compliance in the Valdez regional site.
Assist Nurse, Services Director, and Services Coordinator(s) in ensuring licensing regulations and client health and safety needs are met.
Essential Functions:
Schedules, coordinates, and communicates client medical appointments/follow-up appointments efficiently with other staff ensuring accuracy, personnel coverage, and transportation available.
Must be able to back-up Services Coordinators if they are out of the office/home, ensure managers duties are followed and completed until Services Coordinator returns.
Must be able to escort and care for clients on appointments and overnight trips and be able to work with clients alone or with additional staff.
Must be able to cover shifts in group homes as needed.
Assist Nursing, Services Director, and Services Coordinator(s) in completing and submitting all required documentation, as needed.
Respond to client's questions and needs by editing, canceling and re-scheduling medical appointments as necessary according to FCS protocols; Ensure accuracy of data input with scheduling, provider registration, MAR input, etc.
; Triage calls from Service Coordinators/care coordinators/Direct Support staff to include determining medical emergencies, identifying symptoms, using discretion for work-in physician appointments and staff scheduling; Inventory and refill routine prescriptions and medical supplies for specific clients and deliver to homes.
Update and maintain accurate PMI's and Year at a Glance paperwork with appropriate data input and medical diagnosis information.
Communicate via fax/email/phone/Therap/Alayacare with client guardians, FCS nurse, and other team members all pertinent medical information received from/during doctor's visits and adheres to all follow-up instructions; Maintain, update, and distribute client MAR's with corresponding medications to necessary staff/departments/homes within FCS guidelines.
Work closely with FCS nursing staff, physicians, and therapists to assist in maintaining the comfort and well-being of FCS clients and their families.
Demonstrate accurate and timely documentation; Document client care, reflecting nursing intervention, client response to care provided, client needs, problems, capabilities, limitations, and progress toward goals (documentation to include evidence of appropriate client/staff teaching and the understanding of the instructions noted in the clients' chart).
Maintain up to date information in clients' chart relating to medical appointments and procedures; Notify and document changes in client medical condition to all appropriate parties.
Work effectively within the nursing department in helping organize, file, create and develop systems to better maintain accuracy and efficiency.
Participate in quarterly case file reviews /Participant Monthly Reports/ purge files as necessary.
Maintain health tracking data in Therap system.
Input appointment and health information, monitor data for accuracy, train others on system as needed.
Generate, gather, and maintain required yearly client paperwork, including but not limited to ROIs and HIPAA paperwork.
Provide compliance oversight of assisted living homes in conjunction with the Services Director and Service Coordinator(s).
Perform assisted living home self- inspections as required by regulation.
Gather and report PQI data as requested.
In coordination with Services Director, maintain Safety and Data Sheets (SDS) for Valdez programs.
In coordination with Services Director, ensure assisted living home licensing applications are completed within regulation timelines.
Coordinate with Service Coordinators and assist with preparation of facilities for inspection; including but not limited to, pre- inspection walk-throughs.
Accompany state inspector on yearly assisted living home inspections as needed.
Assist Services Director in completing and submitting and required correction reports to the state; Maintain confidentiality as per FCS policies.
Other duties as assigned.
QUALIFICATIONS, EDUCATION AND EXPERIENCE REQUIRED:
Minimum two years of experience working in the human services field.
Valid LPN or CNA license in the State of Alaska preferred.
Certification in both MANDT and TRANSFER training (agency will train/certify).
Excellent interpersonal, written and communication skills.
A basic knowledge of medical terminology and general body system functions.
Knowledge of OSHA, COA and state regulations preferred.
Excellent telephone etiquette.
Awareness of community resources.
Word, Outlook, and Excel computer skills.
Valid AK Driver's License and automobile insurance.
Personal vehicle to drive between FCS, clients' homes, and Health Services; use of company vehicle takes precedence over personal vehicle for office use, appointments, etc.
PHYSICAL REQUIREMENTS:
Maintain practices, means, methods, operation, or process, reasonably necessary or appropriate to provide a safe and healthy working environment.
Maintain health and safety requirements in keeping with agency policy and procedures.
LICENSES AND SPECIAL REQUIREMENTS:
Position is subject to a background check.
Certified Nursing Assistant license.
Must take TB test.
Must have CPR/First Aid Certification within 30 days of employment (in- house training course available on site).
Maintain infection control procedures.
Disinfection of therapeutic equipment.
