Basic CT scans to include angiography.
If your traveler has worked for any Providence affiliated facility as a perm staff - they must be separated from the health system for a minimum of 6 months to qualify for a travel contract.
Will take first time travelersCandidates permanent address cannot be within 50 miles of facility.

at minimum lead time from Booking to Start to account for Credentialing/Compliance.
SSNDOBRTO must be included at time of the submissionCan only submit to 1 location within the region (PAMC, Seward, Valdez, Saint Elias Specialty Hospital, and Kodiak)Valid State License (Allowed to submit with a pending license, takes 10-12 weeks)Mile Radius Rule Applies ( must be 50 miles from facility)References RequiredNo Locals AcceptedCandidates will float within the hospital or to affiliate hospitals near byCopy of CertificationsSpecific Charting Exp.

If required for the assignment, Client agrees to supply Providers with communication devices (i.

cell phone, pager, etc.
) needed to perform the duties as assigned at no cost to Provider or Agency.
If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of as invoiced by Agency.
Providers will be responsible for submitting monthly cell phone bill directly to Agency.
